Sports Physical Information

*FAMILIES ARE NOT ABLE TO UPLOAD OR EDIT SPORTS PHYSICAL INFORMATION*

Students registering for Athletics must have a current physical examination on file in the Activities Office before the student is authorized to participate in practice. To ensure clearance for participation, physicals must be turned in to the Activities Office no later than 3 school days prior to the first day of practice.

Physicals are valid for 36 months from the date of the exam. Please email completed sports physicals to apettit@detlakes.k12.mn.us
